Unite Us is a free platform for community-based organizations and works with states, payers, and providers. It operates in 44 states and allows users to send or accept electronic referrals, screen for social needs, and collect data outcomes to help drive community investments. In addition, Unite Us integrates with Epic, a company that provides electronic health records (EHR) and other healthcare software. Unite Us works with Epic by integrating with the EHR system, allowing for the exchange of patient information and creating a more comprehensive view of an individual's healthcare journey. The platform aims to address social determinants of health, which are often the root cause of various issues across systems, according to the Vice President of social care integration at Unite Us.
